Test or evaluation of sample under sub-section (4) of section 25 of the Act-(1) The sample of medical device for test or evaluation under subsection (4) of section 25 of the Act shall be sent by registered post in the outer Central Drugs Standard Control Organization, Ministry of Health and Family Welfare, Govt. of India cover addressed to the Director of central medical device testing laboratory in a sealed packet with a memorandum in Form MD-30. (2) The packet as well as the outer cover shall be marked with a distinguishing number. (3) A copy of the memorandum in Form MD-30 and a specimen impression of the seal used to seal the packet shall be separately sent by registered post to the Director of central medical device testing laboratory. (4) After test or evaluation, the result of the test or evaluation shall be sent forthwith to the sender in Form MD-31.
